"Voices of Advance Care Planning"
About the Videos:
| In 2008, the Honoring Choices Minnesota initiative began to take shape in Minnesota. Several key questions were evident as a community-wide approach to Advance Care Planning (ACP) emerged:
Thanks to funding from Aligning Forces for Quality (AF4Q), Minnesota communities and health systems now have a cross-cultural framework that is specific to Latino/Hispanic, Somali and Hmong populations. Each “Voices of Advance Care Planning” video provides under 10-minutes of key messages and talking points for intergenerational users within these communities.
When used in homes, organizations, faith communities, clinics, hospitals and multicultural settings, the messages will motivate viewers to complete their health care directive and share it with their provider as part of their electronic medical record.
Whether you are a patient in a clinic or hospital, a daughter who wants her mother to express her wishes, or an elder facing decision-making, these videos will help to make the conversation easier.
