About the Videos:

 video sidepannel 3images  In 2008, the Honoring Choices Minnesota initiative began to take shape in Minnesota. Several key questions were evident as a community-wide approach to Advance Care Planning (ACP) emerged:
  • “How can sensitive conversations about ACP and end-of-life care best engage communities throughout Minnesota?”
  • “What kinds of education tools and messages would best provide a culturally-sensitive framework for diverse populations?”

Thanks to funding from Aligning Forces for Quality (AF4Q), Minnesota communities and health systems now have a cross-cultural framework that is specific to Latino/Hispanic, Somali and Hmong populations.  Each “Voices of Advance Care Planning” video provides under 10-minutes of key messages and talking points for intergenerational users within these communities.

When used in homes, organizations, faith communities, clinics, hospitals and multicultural settings, the messages will motivate viewers to complete their health care directive and share it with their provider as part of their electronic medical record.

Whether you are a patient in a clinic or hospital, a daughter who wants her mother to express her wishes, or an elder facing decision-making, these videos will help to make the conversation easier.
