Honoring Choices Minnesota's 2nd Annual ACP Thanksgiving Campaign is just the beginning! This campaign encourages all Minnesotans age 18 and over to begin an advance care planning conversation with their loved ones during this Thanksgiving holiday season. There's no better time to begin this intimate dialogue thean when your loved ones are together making memories!

All individuals and organizations are urged to take any of the following actions:
• Post the Top 5 Reasons to Have the Conversation
[download graphic here] on Facebook, your break room bulletin board, or anywhere else you can think of!

Share a postcard [download postcard file here] with others to share ways to help promote our Thanksgiving campaign.

Submit a brief article [download sample template] to your clinic's e-newspaper or other employee/consumer publication on the importance of ACP for all Minnesotans. 

• Submit a brief article [download sample template] to your congregation or faith community's newsletter on including ACP in this year's Thanksgiving holiday gathering. 

• Write a letter to the editor about Thanksgiving being an instrumental time to start this conversation with friends, family and loved onesJoin other physicians and colleagues in making a strong statement this holiday season. We aim for some excellent press coverage.

• Display our ACP Thanksgiving poster (Small - Large) with ACP promotions in your lunch room, break room, lounge, clinic bulletin board or other public places. 

• Watch Twin Cities Public Television stations on Thanksgiving Day for re-airing of our HCM ACP documentariesUrge the elders, young adults and others in your family to view them together. They are excellent "conversation starters" for beginning a lively dialogue between generations.  

• Have the conversation and complete your own health care directive this holiday season. It's one of the greatest gifts you can give your family
