"Healthy Aging NorthEast Coalition Explores ACP Principles"

How do we improve the delivery of health care?  How do we support healthy aging?  These are mountainous questions that the Healthy Aging NorthEast group is investigating in the form of a community dialogue under the guidance of Milton (“Mickey”) Eder, PhD, Dept. of Family Medicine and Community Health and Associate Director of Population and Community Engagement at the U of MN.

Barbara Greene, Director of Community Engagement of Honoring Choices, spoke before 27 participants of this group on September 30 in an active dialogue on ACP community advances and barriers.  During a ninety-minute dialogue, the group discussed concerns around chronic disease management, engaging young persons in the conversation, reframing the dialogue to be about quality of living, and institutional barriers to the conversation.  Participants ranged from educators at the Basilica of Saint Mary in Minneapolis to staff of Northeast Bank, a gerontologist from the U of MN Dept. of Family Medicine and Community Health, a doctoral student at the U of MN School of Public Health (Epidemiology), human service professionals from many community-based organizations and interested individuals from the community.

Funded by a national Patient Centered Outcomes Research Institute (PCORI) grant to learn “what works best” within communities, Healthy Aging NorthEast brings together individuals, organizations, businesses, health officials, faith communities, and other interested stakeholders from the community.  One of their key goals is to engage the community through informed discussions that identify ways to promote healthy aging.
