     Multicultural Tools

Voices of Advance Care Planning:  Join the team behind (and in!) the new videos Honoring Choices recently released. 

In addition to viewing all three videos, you will hear the story of why the videos were developed, how the content was designed, who the people are that you see in the videos, and how they are already being used. 

        Where Are They Now?

Allina, Fairview, HealthPartners and Park Nicollet have been building successful ACP programs in their systems for several years now. 

Listen as representatives from each system share the stories of their internal program development - how their strategies and goals have changed over time, and where they are today.

          Spiritual Perspectives
Keynote speaker Ruth Bachman leads this session focused on helping people learn how to grow, spiritually, through times of challenge and difficulty. 

Ruth will provide a roadmap of sorts, encouraging participants to look closely and patiently at all of life's challenges - narrow spots - to discover the resources, both internal and external, that are present to help navigate the passage.  These resources are helpful for both personal use and as tools to assist patients, clients and others.

            ACP in Greater Minnesota
Though Honoring Choices began in the metro area of Minneapolis and St. Paul, efforts today encompass all Minnesotans.  As we expand and grow, we are learning how different strategies are often needed to meet the needs of communities that are building ACP programs away from the Twin Cities. 

Representatives from several communities will share what has worked well, what challenges have been dealt with, and answer your questions. 

In addition, attendees will leave with tools, information and tips about building community support for a new idea, chosen to help anyone interested in starting or growing an ACP program.
