WHAT IS NHDD National Healthcare Decisions Day (NHDD) is a nationwide initiative committed to ensuring that all adults with decision-making capacity have the information and opportunity to communicate and document their healthcare decisions. On April 16th every year, hundreds of NHDD events are held around the country. In Minnesota, attendees of NHDD events will have the opportunity to learn more about advance care planning and the importance of having a conversation with family and loved ones. There’s no “right” or “wrong” to advance care planning. The most important thing is to simply begin the conversation with those you care about and trust. It is the hope of Honoring Choices Minnesota that providing a platform where organizations can publicize and promote their respective events will help engage and impact communities throughout our state. Our website at www.honoringchoices.org is a huge resource for Minnesotans. Thank you for getting the word out and promoting the diverse advance care planning resources that we have collectively created. HISTORY AND PURPOSE OF NHDD In 2008, both houses of Congress formally recognized National Healthcare Decisions Day (S.Con.Res. 73 & H. Con.Res. 323), officially establishing the NHDD event as a national initiative to occur each April. Own Your Own Future is a Minnesota initiative that urges all residents to have a plan for their long-term care, including how to pay for it. Own Your Own Future is a state/federal partnership to educate and encourage all Minnesotas to get the facts and plan for their long-term care.
Honoring Choices Minnesota is a partner of Own Your Future and promotes advance care planning as one aspect of a personal long-term care plan.
- Provider Orders for Life-Sustaining Treatment (POLST)
A Provider Orders for Life Sustaining Treatment (POLST) form contains medical orders for patients thought to be near the end of their life. It addresses many of the same things as a health care directive, but would typically be completed by the provider, rather than the patient.
