Purchase a Sub-License PDF Print E-mail

As a healthcare organization you have the option of purchasing a sub-license agreement. There are two sub-licenses available for purchase. Read more on the two types by following this link. 

1) Complete one sub-license agreement, either on-line or through the mail:

                 proofs of the materials with your logo for approval.   

            > Materials Purchase and Sub-License Agreement: 
                 Download form to complete and mail     ||    Complete form & pay online                         

2) You will receive confirmation from Honoring Choices Minnesota that we have received your completed sub-license agreement and payment.
