The 3rd annual Sharing the Experience: Honoring Choices Minnesota Conference will be held on Thursday, July 19, 2012.  Register online. Health care providers, community members, Ambassadors, long term care workers and volunteers are all encouraged to attend.

Attendees will learn the progress and findings of featured advance care planning programs, hear updates from Honoring Choices Minnesota partners and insights from guest speakers, and network with others who are passionate about promoting advance care planning conversations in MN. 2011 Sharing the Experience Conference
The 2011 Sharing the Experience: Honoring Choices Minnesota Conference was held on July 20, 2011, in Minneapolis. 

It was an inspiring event, as representatives from each local advance care planning program made a presentation on their progress.  Their PowerPoint slides can be viewed below.

2010 Sharing the Experience Conference 
The first Sharing the Experience Conference was held in 2010 to present the outcomes and learnings of the first Honoring Choices Minnesota advance care planning pilot teams. Pilot Progam Summary document.

The teams presented reports summarizing their work from Jan-June 2010 -- those PowerPoint presentations can be viewed below.


Representatives from Allina and Park Nicollet also presented updates on their implementation of system wide advance care planning standards:
