I was on call for my group of ICU doctors Wednesday night when I was called by a nurse and told that a new patient was just being transferred to the ICU in severe respiratory distress. The patient was an elderly gentleman suffering a neurological condition that caused progressive swallowing difficulty - to the point that he had to be fed by a gastrostomy feeding tube for several years. In spite of this he had been recurrently aspirating, developing pneumonias and other breathing difficulties. |

 Graceful Journey, a pilot project by the Minnesota Council of Churches in partnership with Honoring Choices Minnesota, is designed to help people think about the kinds of decisions that need to be made at end of life, to offer the tools necessary to discuss and make those decisions, and to encourage congregations to provide the care needed to make end of life a graceful journey for its members. |

Advance Care Planning Facilitator Training |

Basic Advance Care Planning (ACP) Facilitator training is for any interested individual who wants to learn how to lead in-depth and effective ACP conversations with others. This training is the Respecting Choices curriculum and certification. (details on the Respecting Choices approach).
TCMS is exploring options for ACP facilitator training later in 2013. Park Nicollet also offers both Basic Facilitator Training ("First Steps") and POLST Facilitator Training ("Last Steps").
